7th Grade Required Immunizations

Nevada State Law requires that a student receive the MCV4 (Meningococcal) vaccination and the Tdap vaccination prior to entering 7th grade. If your child has not yet received the required MCV4 and/or Tdap immunization(s), please take the flyer below along with any existing immunization records to your health care provider or the Southern Nevada Health District to obtain the required vaccine(s). The Southern Nevada Health District’s clinic sites can be found at http://www.southernnevadahealthdistrict.org/

Provide a copy of the updated immunization record prior to the first day of school. If your child is exempt from any immunization(s), please provide the necessary documentation. Should you have any questions or require additional information, please call the health office at 702-799-0920 X4022.
